Numbered Memo 2012-10: Non-Voting Systems Incident Reports

The non-voting systems incident report form has been changed because of pending redistricting litigation.

Attached is a revised version of the form that appears at pages 14-29 and 14-30 of the NC Elections Precinct Uniformity Project. This form has been changed because of pending redistricting litigation. Thus, use of this form is mandatory

The substantive change in the form is the addition of:

  • Incident Type checkboxes
  • Additional selections to indicate the nature of the incident.
    • Voter Offered Provisional Ballot but Chose Not to Vote Provisionally
    • Voter Assignment Issue
    • Ballot Issue - Voter Given Incorrect Ballot
    • Ballot Issue - Correct Ballot Unavailable

It is important to distinguish between those matters that arise prior to Election Day and those that arise as a result of a person presenting to vote on Election Day. Please note that voters choosing to vote out of precinct do not give rise to the need to record an incident report.

Please encourage your precinct workers to complete this report with as much detail as possible so that any incidents identified may be resolved. Information about the circumstances that gave rise to the incident is particularly important to include so that the problem can be prevented in the future.
