Numbered Memo 2012-08: 2012 Wellness Check

The wellness check and electronic submission of all documents must be completed by Tuesday, July 31, 2012.

In advance of the 2012 general election, each county will be required to complete a self-administered Wellness Check and electronically submit of a series of supplemental documents. With all the happenings in 2012, there will be intense scrutiny on our elections processes. Ensuring compliance of all law and processes is a top priority to avoid problems in November.

The wellness check will be conducted via an online survey. In the near future, each county will be emailed a link to its own survey, which can be saved midway to be completed in multiple sittings. As with past wellness checks, this will be a comprehensive review of numbers, procedures and compliance within each CBE office.

Along with the email with the link to the individual survey, a cover sheet will be provided requesting a number of documents (sample attached). The cover sheet includes instructions to send the documents to the SBE office via fax. A statewide inventory of these faxed documents will be kept as PDF files. Submission of the documents must be done electronically by fax — do not mail the documents to the SBE office. The wellness check and electronic submission of all documents must be completed by Tuesday, July 31, 2012.

Upon review, some wellness checks may require in-person interviews to follow up on responses. If the survey and documents are not timely received or incomplete, an in-person audit by SBE staff may be necessary.
